Daily Function Check In

Notice which areas of daily functioning feel supported, changed, or in need of attention without assigning yourself a score or grade.

Before you begin

This is a check in, not a test.

Low mood can affect sleep, nourishment, hygiene, movement, medication routines, responsibilities, and connection. Choose the response that best describes today.

Not every category applies to every person. Disability, chronic illness, culture, resources, work demands, and living circumstances may shape what daily care looks like.

Take medication only as prescribed. Contact your prescriber or pharmacist with medication questions rather than changing or stopping medication on your own. Seek professional support when changes are persistent, significant, or interfering with daily life.
